The Menopause Connecting Care Clinic (MCCC) is a service helping females experiencing hormonal problems to access a broad and supportive clinical team to support menopause related care. Our aim is to work closely with the patient and their usual GP / Doctor to achieve their menopause goals.

Clinical service overview

MCCC is available to females with hormone related conditions requiring a multidisciplinary team approach to their care. The clinic is located at Sandringham Hospital.

The supportive MCCC team will:

  • Ascertain menopausal stage, evaluate symptoms and initiate appropriate care
  • Provide a clear care plan to help bridge the gap for women and primary health care providers

The team includes:

  • Endocrinologist
  • GPs with a special interest in menopause
  • Nurse Practitioner
  • Physiotherapist
  • Dietitian
  • Psychologist

This clinic is designed to provide multidisciplinary assessment, over a maximum of 3 medical consultations, to develop a care plan to bridge the gap for women and primary health care providers. We will transfer care back to their usual GP on discharge.

Who we care for

This clinic is available to people with female-specific hormone-related health issues. This includes:

  • Perimenopause
  • Menopause
  • Premature ovarian insufficiency
  • Irregular or absent menses potentially due to hormonal changes
  • Peri/menopause-associated sexual dysfunction

This clinic does not manage:

  • Termination of pregnancy
  • Infertility
  • Sexually transmitted disease
  • Gynecological conditions, including undiagnosed vaginal bleeding and IUD insertion

  • Completed referrals are sent securely to the department or clinic selected.
  • You will receive an acknowledgement of receipt and progress updates on your referral
  • A copy of the referral will automatically be stored securely in the patient’s record.

eReferrals

We accept GP and specialist referrals to this service. 

All referrals are triaged by the service according to clinical urgency. Patients requiring immediate assessment should be sent to the Emergency & Trauma Centre

To refer your patient, please send an eReferral. For urgent referrals, also contact the service Registrar to discuss the case.

To ensure appropriate and timely triage, include all demographic and clinical details as well as relevant investigation results. 

If you are concerned about any delay of the appointment or if there is any deterioration in your patient’s condition, contact the service Registrar on call on (03) 9076 2000.
