The Diabetes Connecting Care Clinic (DCCC) is a service helping people with Type 2 diabetes to access a broad and supportive clinical team. Our aim is to work closely with the patient and their usual GP / Doctor to achieve their diabetes goals.

Clinical service overview

DCCC is available to adults with type 2 diabetes and suboptimal glucose levels (HbA1c >8%), requiring a team approach of different specialists to support their self-management. The clinic is based at the Caulfield Hospital.

The supportive DCCC team will:

  • Help to improve their understanding of type 2 diabetes
  • Provide clear guidance and instructions to patients to help self-manage their diabetes
  • Help patients to learn how to monitor their diabetes, and reduce the risks of developing diabetes complications

The team includes:

  • Endocrinologist
  • GP Specialist
  • Pharmacist
  • Diabetes Nurse Educator
  • Dietitian
  • Exercise Physiologist

After a series of appointments over 6 to 12 months, and once the person with diabetes has achieved their goals, we will transfer care back to their usual GP through a virtual case conference.

  • Completed referrals are sent securely to the department or clinic selected.
  • You will receive an acknowledgement of receipt and progress updates on your referral
  • A copy of the referral will automatically be stored securely in the patient’s record.

We accept GP and specialist referrals to this service. 

All referrals are triaged by the service according to clinical urgency. Patients requiring immediate assessment should be sent to the Emergency & Trauma Centre

To refer your patient, please send an eReferralFor urgent referrals, also contact the service Registrar to discuss the case.

To ensure appropriate and timely triage, include all demographic and clinical details as well as relevant investigation results. 

If you are concerned about any delay of the appointment or if there is any deterioration in your patient’s condition, contact the service Registrar on call on (03) 9076 2000.

MBS-billed service

On the doctor’s behalf, Alfred Health can bulk bill the cost of the consultation for some services through Medicare. This means there is no out of pocket expense to the patient. 

MBS-billed services require a current referral. Provide your patient with a 12 month referral to see the specialist of your choice at the requested clinic. 

Referrals must be addressed to a named specialist or the Head of service, listed at the top of this page. Clinic consultants are listed on every clinic page.

Note that from time to time, your patient may be seen by another specialist in the clinic, in order to expedite their treatment.
