Better at Home is a home-based rehabilitation program modelled on an inpatient subacute ward. It offers an alternative approach to delivering inpatient care and treatment to people within their own home environment.
Clinical service overview
We offer specialist assessment and therapy from Geriatricians and Rehab Physicians, Nurses, Pharmacists, Dietitians, Occupational Therapists, Physiotherapists, Social workers, Speech Pathologists and Allied Health Assistants through a goal orientated, time limited program.
Patients admitted to Better at Home are classified as an inpatient of Caulfield Hospital. They receive between one and four home visits per day from members of our interdisciplinary team depending on the individual’s goals, and access to 24 hour telephone support. The average length of stay on Better at Home is 2 weeks and during this period our patients receive active case management and discharge planning.
Patients must reside within the Better at Home catchment area.
Typical therapies and treatment offered on Better at Home program include:
- Geriatric assessment
- Pain management
- Falls risk assessment
- Chronic illness management
- Medication review and management
- Wound care
- Catheter management
- Blood monitoring including warfarin
- Daily subcutaneous IM/IV medication administration
- Rehabilitation post orthopaedic surgery
- Physical therapy
- Home equipment assessment
- Personal care (limited)
- A ‘Welcome Home’ meal is offered to those who it is indicated for.

We accept GP and specialist referrals to this service.
All referrals are triaged by the service according to clinical urgency. Patients requiring immediate assessment should be sent to the Emergency Department.
To refer you patient, complete and fax a referral to us. For urgent referrals, also contact the service Registrar to discuss the case.
To ensure appropriate and timely triage, include all demographics and clinical details as well as relevant investigation results.
If you are concerned about any delay of the commencement of the service or if there is any deterioration in your patient’s condition, please contact the service immediately on 0439 526 869.
We accept referrals for any Alfred Health inpatients.

Under Orders & Referrals search for GEM at Home and complete the required information.
Inpatients at The Alfred and Sandringham Hospitals will be reviewed by the Rehab and Aged Care Service (RACCS) team.
Inpatients at Caulfield Hospital will be reviewed by the Better at Home Clinical Coordinator.
