What we do
Our service provides access to a full range of diabetes therapies, including continuous glucose monitoring devices and insulin pumps.
What to expect
Appointments are offered in person (telehealth considered on an individual basis). You may see multiple clinicians on the same day. The team will provide you with advice and guidance for managing your type 1 diabetes, and will also communicate with your treating GP.
How to access this clinic
Referral from your GP or specialist
You need a referral letter from your GP or medical practitioner to access this service.
Your doctor will need to fax or email your referral letter to us. We will be in contact with you in clinically recommended times, depending on waiting list length. If there is no waiting list, you will receive an appointment booking letter or we will contact you to arrange a suitable time.
What to bring
For a clinic appointment
- Your appointment letter
- Any special items listed on your letter
- Medicare card
- Your diabetes medicines
- Your blood glucose meter
- Your glucose diary/record
- List of all your current medicines (or the boxes), including over-the-counter medicines such as vitamins and herbal supplements
