Our multi-disciplinary team are highly specialised in addressing and managing the medical, social and psychosocial issues relating to cystic fibrosis.

Clinical service overview

The Alfred Cystic Fibrosis Service provides specialist multi-disciplinary care for 240 adults who attend more than 1,000 outpatient appointments per year.

We have around 250 inpatient admissions per year of people requiring more intensive therapy, which includes access to the Hospital in the Home program.

The service is actively engaged in clinical and basic research centred on host organism interaction, airway remodelling, nutritional needs, airway function and psychosocial issues in cystic fibrosis.

Your referral should include at a minimum:

  • Patient demographic information
  • Referrer demographic information
  • The reason for referral
  • The presenting problem
  • The service(s) requested
  • Required referral information specified by statewide or local referral criteria
  • Current patient management

Provide if available: 

  • Spirometry
  • Height
  • Weight
  • Sputum culture results
  • Echocardiogram report (including date and details of the diagnostic imaging practice)
  • Chest CT scan (including date and details of the diagnostic imaging practice)
  • Current vaccination status
  • Details of any Advance Care Directive
  • If the person identifies as an Aboriginal and/or Torres Strait Islander

  • Completed referrals are sent securely to the department or clinic selected.
  • You will receive an acknowledgement of receipt and progress updates on your referral
  • A copy of the referral will automatically be stored securely in the patient’s record.

eReferrals

We accept GP and specialist referrals to this service. 

All referrals are triaged by the service according to clinical urgency. Patients requiring immediate assessment should be sent to the Emergency & Trauma Centre. 

To refer your patient, please send an eReferral. For urgent referrals, also contact the service Registrar to discuss the case.

To ensure appropriate and timely triage, include all demographic and clinical details as well as relevant investigation results. 

If you are concerned about any delay of the appointment or if there is any deterioration in your patient’s condition, contact the service Registrar on call on (03) 9076 2000.

Special referral instructions

If the patient is transferring from another Cystic Fibrosis Care Service, please send a medical letter which details the care given at that Centre and the patients current health status, as well as copies of any previous scans and test results. Enquiries about delays in receiving appointments go to the Clinic Coordinator (03) 9076 6960. Calls to discuss patient deterioration go to the Registrar via AH Switchboard on (03) 9076 2000 or the CF Nurse on (03) 9076 3443.

MBS-billed service

On the doctor’s behalf, Bayside Health can bulk bill the cost of the consultation for some services through Medicare. This means there is no out of pocket expense to the patient. 

MBS-billed services require a current referral. Provide your patient with a 12 month referral to see the specialist of your choice at the requested clinic. 

Referrals must be addressed to a named specialist or the Head of service, listed at the top of this page. Clinic consultants are listed on every clinic page.

Note that from time to time, your patient may be seen by another specialist in the clinic, in order to expedite their treatment.
