Aged Care Assessment (ACA)

We are a multidiciplinary team that conducts independant assessments for older people to determine eligibility for a range of Australian Government funded aged care services and supports.

The way aged care assessments are delivered is changing. Across Melbourne's south-east, health services are working more closely together under a new assessment model led by Monash Health to provide a more coordinated and consistent experience for older people, families and referrers.

Bayside Health Alfred will continue to play an important role in delivering timely, high-quality assessments that support older people to access the care, services and support they need.

Services we offer

We are a free service conducting Comprehensive assessments and Home Support assessments for people who need to access community or residential aged care services.

The assessment takes place in the person’s home setting or in the hospital and assesses eligibility to access Commonwealth funded services:

Residential aged care
Residential respite care
Transition Care Program (TCP)
Restorative Care Pathway (RCP)
Support at Home (SAH)
Commonwealth Home Support Services (CHSP)
End of Life Pathway (ELP)

The assessment also:

Involves clients, their support people and service providers in the assessment and care planning process
Arranges interpreters to assist in the assessment process if required
Provides information, referrals and liaising with other aged care services
