Horizon Place is a supported accommodation service for people living with HIV. Horizon Place provides a non-medical environment with 24 hour support via attendant carers.
Clinical service overview
There is a mixture of shared accommodation and independent flats. Horizon Place is located in East Melbourne.
RDNS provide nursing care to residents in Horizon Place as they would for individuals living in the community.
The service is for individuals who can no longer live independently in the community because of a range of health and/or social issues.
In addition to long term care the service also provides a range of short and medium term accommodation options for:
- people transitioning from hospital care back to independent living in the community
- carer and social respite
- people living in regional areas visiting Melbourne for medical treatment
- limited overnight accommodation where there is no access to emergency accommodation where vacancies exist
