Inner South East Post Acute Care

The Inner South East Post Acute Care (ISEPAC) program aims to support patient's recuperation after a hospital episode, via the provision of individually tailored packages of community based care services.

PAC arranges support services in a timely, responsive manner and respects cultural values.

The program promotes health and independence whilst providing a vital link to ongoing community supports.

Where another program was providing supports prior to the hospital episode it is expected that these services will be resumed on the person's discharge to home and PAC can supplement that service.

Our Service

  • Provides expertise in discharge planning to provide a safe and smooth transition from hospital to the home.
  • Funded short term services usually up to 28 days or until the long term program is available (eg. HACC, CACPs, Linkages, Disability Services)
  • Ensures a holistic approach to assessment.
  • Coordinate with the patient when planning the discharge and arranging support services.
  • PAC is available for all ages groups
  • Monitor patient's post discharge and provide reactive care coordination as required

Description of PAC support services include:

  • Nursing care
  • Physiotherapy
  • Personal Care
  • Home care
  • Shopping (accompanied or unaccompanied)
  • In home respite
  • Escorts home from hospital (in selected circumstances)
  • Case Management

The ISEPAC program, as part of the Inner South Community Health Service, provides services to all public hospital patients residing in the municipalities of City of Port Phillip, Stonnington, Glen Eira and Bayside.
