At Alfred Health we value and welcome input from the community about how we can improve the quality and safety of our services.
As we to strive to do better, feedback from the community and consumers helps us address the diverse needs of patients, carers and the communities we serve.
Alfred Health Community Participation Model
The Alfred Health Community Participation Model has been developed as a tangible expression for the organisation's commitment to Community Participation and engagement.
The model is based on the strategic goal of ‘Working in partnership with our community, consumers and carers to improve our services' (Alfred Health Strategic Plan 2006 - 2010) and the 5 elements that are incorporated into this goal:
These 5 elements are the foundation of the Community Participation Plan and the model illustrates how they address the 3 key levels of the health service system; Individual Care Level, Program or Department Level and Organisational Level.
The Key Performance Indicators of the Community Participation Plan are depicted in the model as ‘Measures of Success' and again, are aligned with the relevant level(s) of the health service.
Community Participation Model Launch
The Community Participation Plan is developed by the Alfred Health Community Advisory Committee in consultation with health service staff. The current 3 year Plan was endorsed by the Alfred Health Board in September 2006 and progress is monitored annually by the Community Advisory Committee.
