We Care about your feedback

Quality services are important to us.  We care about your views and experience and value your feedback.

If you have a compliment, complaint or general feedback you may:

  • Communicate with the relevant staff member directly or communicate with a senior staff representative
  • Provide your feedback in verbal or written form via face to face contact, phone on 9076 6666 or by letter or email to cchs@cgmc.org.au
  • Contact the Caulfield Hospital Complaints Liaison Officer on 9076 6127
  • Complete a "We Care" form available in each of the CCHS reception areas or download here

You will find a box in each of the reception areas where you can anonymously lodge your form. The form is then sent to our Complaints Liaison Officer who will contact you to discuss the matter within 24 hours of receiving the feedback.

If required, an investigation will be undertaken and a response will be sent to you within 10 working days (where possible).

If the matter remains unresolved or you are still dissatisfied, you can contact the Health Services Commissioner, 30th Floor, 570 Bourke Street, Melbourne 3000.
