Outreach Pharmacy
Access to Outreach Pharmacy

The Pharmacy Outreach Service aims to assist with optimising medication use by providing education, recommendations for medication management and review of medications for drug interactions, adverse effects and optimal drug choice.

An experienced Clinical Pharmacist visits HARP patients at home and community in DMU outpatient clinics. The pharmacists liaises between hospital and community providers to improve continuity of care in order to prevent medication misadventure and hospital readmission.

Outreach Clinical Psychlogist
The HARP Clinical Psychologist provides a range of clinical psychology services to Alfred Health HARP services.

This potentially includes 

  • primary care (eg. Psychological assessment, brief intervention with anxiety, depression, grief/loss, adjustment to illness or adherence to treatement),
  • secondary consultation (eg. Re psychological aspects of client's presentation, information re referral options),
  • tertiary service (eg contributing to professional development of HARP staff). The Clinical Psychologist also coordinates the self management work group for HARP and works closely with key partnering agencies across the Alfred Health community.

Guidelines for referrals are currently being finalised.
