Description of Service
The Home-Based program provides clients with short-term, intensive, home-based rehabilitation for a maximum period of 8 weeks.
The program assists clients who are having difficulty performing their daily activities following an accident, injury or medical condition.
There are no fees charged for this service.
The home-based team includes:

Physiotherapy
-
Occupational Therapy
-
Speech Therapy
-
Social Work
-
Community Health Nursing
-
Dietetics
-
Allied Health Assistant
-
Rehabilitation Medicine
-
Neuropsychology
ELIGIBILITY:
-
Residents must reside in the following council areas:

Live at home,hostel or SRS which is suitable for the provision of a rehabilitation service.

Be medically stable and have a local GP responsible for their medical care.

Must have had a recent illness, injury, or medical event resulting in a change in level of functioning.

Be 18 years of age or over.

Be able to actively participate in a rehabilitation program.

Have home-based rehabilitation goals.
EXCLUSION CRITERIA:

Individuals who are able to travel into a facility for rehabilitation. Being unable to afford transport or not having own transport does not make individuals eligible.

Individuals who have cognitive deficits or psychiatric conditions rendering them unable to participate or benefit from the program.

Individuals who require ongoing maintenance therapy for chronic conditions.
- Referrals for home modifications only will not be accepted.

Individuals who require an occasional session of therapy post hospital discharge to check safety concerns or equipment.

Individuals living in a nursing home.
