Caulfield Community Rehabilitation - Centre-Based Service

Description of Service

The Centre-Based program provides clients with short-term, intensive, centre-based rehabilitation for a usual period of 8 weeks.

The program assists clients who are having difficulty performing their daily activities following an accident, injury or medical condition

The centre-based team includes:

  • Physiotherapy
  • Occupational Therapy
  • Speech Therapy
  • Social Work
  • Community Health Nursing
  • Dietetics
  • Allied Health Assistant
  • Rehabilitation Medicine
  • Neuropsychology 

Group and individual therapy is offered. Groups are run by Physiotherapists, Occupational Therapists and Speech Therapists and target:

  • Strength
  • Balance
  • Community Access
  • Communication
  • Hydrotherapy

Fees:

$7.00 for pension or concession card holders

$15.50 for non-concession card holders.

Eligible clients will not be denied a service because of an inability to pay.

Transport:

Clients are expected to organise their own transport. Should a taxi be required, a booking can be made at clients expense.

ELIGIBILITY:

  • Residents must reside in the following council areas: Glen Eira, Stonnington, part of Bayside
  • Be medically stable and have a local GP responsible for their medical care.
  • Must have had a recent illness, injury, or medical event resulting in a change level of functioning.
  • Be 18 years of age or over.
  • Must have identified rehabilitation goals which are best achieved in a centre-based facility and may benefit from interdisciplinary input.
  • Be able to actively participate in a rehabilitation program.
  • Must be willing and safe to travel into the centre via car/taxi and able to mobilise a minimum of 50m. (this includes wheelchair/scooter users)
  • Agree to participate, live in catchment area

GENERAL EXCLUSIONS:

  • Individuals who have acute injuries requiring one on one, cubicle based treatment
  • Individuals who have cognitive deficits or psychiatric conditions rendering them unable to participate or benefit from the program
  • Individuals who require ongoing maintenance therapy for chronic conditions
