Horizon Place
Description of Service
Horizon Place is a supported accommodation service for people living with HIV. Horizon Place provides a non medical environment; it has 24 hour support via attendant carers.
There is a mixture of shared accommodation and independent flats. Horizon Place is located in East Melbourne.
RDNS provide nursing care to residents in Horizon Place as they would for individuals living in the community.
The service is for individuals who can no longer live independently in the community because of a range of health and/or social issues.
Assessment of individual's need for this type of care is undertaken through Fairfield House.
In addition to long term care the service also provides a range of short and medium term accommodation options for:

People transitioning from hospital care back to independent living in the community.

Carer and social respite.

People living in regional areas visiting Melbourne for medical treatment.

Limited overnight accommodation where there is no access to emergency accommodation where vacancies exist. (Referral to be made by one of The Alfred's Social Worker or AHAG required.)
