HARP MATS at Sandringham is part of the Alfred Health HARP which provides service to persons who have chronic conditions and use Alfred Health frequently by attending Emergency Departments or inpatient units.

Our service has a variety of ways of delivering the best care to assist in improving disease management, and support the person to be self managing their own health needs. We work both within the hospital setting, and also in the community health services and can visit to home.
