Therapy In The Home
Description of Service
The program provides short-term intensive rehabilitation programs for clients, within their own homes and communities.
Treatment programs aim to assist clients and carer/s achieve their rehabilitation goals. The multidisciplinary team visit clients at home for up to 8 weeks.
Home based therapy allows clients to develop independence with daily activities in a relevant setting, allowing carers and families to be closely involved in the rehabilitation process.
Clients must reside within the local government areas of: Glen Eira, Stonnington, Port Phillip and Bayside.
