Description of Service

Social Work aims to identify and address the psychosocial impacts of illness and hospitalisation for patients and their families by providing a range of services which increase quality of life and participation in care.

Social Work also assists patients and their families with a smooth transition from hospital to home.

Social Work provides a range of services including:

  • Counselling
  • Discharge planning
  • Crisis interventions
  • Bereavement counselling
  • Family support
  • Care coordination
  • Making appropriate referrals
  • Community liaison

Groups are also facilitated in areas such as Brain Tumour Support, Living with Cancer, Cardiac, Dialysis, Burns and Diabetes Education Group.Social Work provides services in a range of locations and clinical specialities including:

  • Trauma - Emergency, ICU, Burns, TAC, Hyperbaric
  • Infectious Diseases and HIV/AIDS
  • Cardiology including Transplant and Cardiothoracic
  • Cancer - Bone Marrow Transplant, Radiotherapy, Haematology and Palliative Care
  • General Medical Units
  • Aged care and Neurosciences
  • Renal and Cystic Fibrosis

In 2000, Social Work Services provided 20,031 occasions of service to 4,050 Inpatients and 6,609 occasions of service to 1,528 Outpatients.

 

Alfred Drink Drive Course

The Alfred Drink Driver Course is an eight-hour education session to help facilitate individuals in having their licence restored.  Our course is run over two weeks on Wednesday evenings.  We are a Victorian Department of Health accredited agency (No. 97/055), a member of Victorian Association of Drink Driver Services and will provide participants with a certificate upon successful completion of the course.  Our agency is able to provide further licencing assessments should these be required.  Further assessments are determined by the severity of your drink driving offence.  Licence eligibility assessments and interlock assessments can be booked through further discussion with our agency.

Middle Park, Carlton and Prahran Market G.P. Clinics as well as the Positive Living Centre through the Infectious Diseases Unit
